社區為本服務之協作 導航計劃與多個社區精神健康及社會服務機 構建立緊密協作關係 ， 以為精神復元人士在 過渡期階段 ， 建立全面且適切的支援 ， 增強支 援網絡 。 本計劃成功協助共計 61 人次連結至 不同的社會及醫療服務單位 ， 包括精神健康 綜合社區中心 、 庇護工場 、 職業康復中心 、 殘 疾人士就業及培訓服務 、 家長 / 親屬資源中心 、 綜合家庭服務中心 、 地區康健康中心以及過 渡性房屋等 。 Community-Based Service Collaboration LHP has established close collaborative relationships with various community mental health and social service organisations to provide comprehensive and appropriate transitional support for users, thereby strengthening their support networks.
